Please bring any relevant information to your first appointment, including a copy of your auto insurance card, a claim number, and a police report if you filed one. Our office direct bills insurance and helps you through all the paperwork. We can email or text the forms to you to print or fill out online.
What to Expect
Your customized plan of care may include chiropractic adjustments, physical therapy, Active Release Technique®, and muscle work. Dr. Jerod and Dr. Evan focus on restoring the normal curvature of your spine, while also helping your muscles relax. If you need extra support, we can refer you to a great local physical therapist or massage therapist. We take a teamwork approach to connect you with the resources you need for your recovery.
